INDEMNITY-RELEASE-DISCLOSURE AGREEMENT

 

The following information will be used for insurance purposes. However, said insurance does not relieve any rider from personal liability for her/his negligent acts. Due care should be taken at all times to safeguard the public, fellow riders, and the individual rider. The Krewe of HBCUs reserves the right to subrogate any insurance claim against the person giving rise to said claim. This membership form is not valid if the ACCEPT box is not checked at the end of the form. All payments made to this organization are non-refundable and non-transferable. Cancellation of the parade by the City of New Orleans or due to unsafe weather conditions does not give rise to a refund.

 

I, the undersigned, agree and due hereby release KREWE OF HBCUS, the City of New Orleans, their representatives, employers, officers, and volunteers from any liability for the injury or harm to myself which may occur as a result of my participation in its Mardi Gras parade and activities. I have been advised of all necessary safety precautions and I have been informed that there can be no guarantee that my participation in the parade is entirely risk free. I, therefore, agree to assume all responsibility for the risk of any injury that I might sustain. I also understand the need for and agree to wear the positioning device (harness), selected by the Krewe of HBCUs, at all times while participating in the parade. I further agree that if an act of God, or any other circumstances beyond our control occurs, no refunds or credit on dues will be given. I acknowledge that I have had the opportunity to ask any questions concerning safety precautions to be taken within the parade and/or activities. Any member of the Krewe of HBCUs organization caught not wearing their complete costume and/or mask during the parade will be assessed a fine of up to $200.00 for the first offense. For any subsequent offense you can be removed from the Krewe. Should you not be able to participate once a member, a $150.00 replacement rider fee will be assessed and must be approved by the Board.

 

I will agree to all rules and regulations of KREWE OF HBCUS if I am accepted for membership. I agree to release the KREWE OF HBCUS, its officers, employees, directors, agents, and members from any and all damages or losses sustained by myself as a result of my participation in the parade and related activities. I understand that this membership is non-refundable if I am no longer able to ride. I also understand that this membership is non-transferable unless agreed and approved by the Board of Krewe of HBCUs.
